What to Expect at Your First PT Visit

Walking into a physical therapy clinic for the first time can feel uncertain. What will they do? Will it hurt? How long will it take? Here’s exactly what happens so there are no surprises.

Before Your Appointment
What to bring:
- Your insurance card
- Photo ID
- Referral from your doctor (if your insurance requires one; check here)
- List of medications you take
- Any imaging results (X-rays, MRIs) if you have them
- Notes about your symptoms: when they started, what makes them better or worse
What to wear:
- Comfortable, loose-fitting clothes
- Shorts if your knee, hip, or ankle hurts
- A tank top or loose shirt if your shoulder or neck hurts
- Sneakers or athletic shoes (not flip-flops)
Your PT needs to see and touch the area that hurts, so wear clothes that allow easy access.
The Evaluation (45-60 Minutes)
Your first visit is mostly an evaluation. Here’s what your PT will do:
1. Ask Questions (10-15 minutes)
Your PT will ask about:
- Where it hurts and when it started
- What activities make it better or worse
- Your medical history and past injuries
- Your goals (back to running? Pain-free at work? Playing with your kids?)
- Any previous treatments you’ve tried
Be honest and specific. “It hurts when I go down stairs” is more helpful than “it just hurts sometimes.”
2. Physical Assessment (20-30 minutes)
Your PT will test:
This shouldn’t be painful, though some tests might reproduce your symptoms briefly. That’s actually helpful because it tells your PT what’s going on.
3. Treatment Plan (5-10 minutes)
After the evaluation, your PT will explain:
- What they think is causing your pain
- How many sessions you’ll likely need (timelines vary by condition)
- What exercises and treatments they’ll use
- What you can do at home between visits
Will They Treat You on the First Visit?
Usually, yes. After the evaluation, most PTs will start some initial treatment:
- Manual therapy. Hands-on techniques to improve joint mobility and reduce pain
- A few key exercises. You’ll learn 2-4 exercises to start your home program
- Pain management. This might include ice or heat, electrical stimulation, or taping
- Education. Tips on what to avoid and how to manage symptoms between visits
You won’t get your full exercise program on day one. That builds over the first few visits as your PT learns more about how your body responds.
What Happens at Follow-Up Visits?
Follow-up visits are typically 30-45 minutes. A typical session includes:
- Quick check-in on how you’re feeling
- Hands-on treatment (manual therapy, dry needling, etc.)
- Guided exercises with your PT watching your form
- Updates to your home exercise program
- Foam rolling or stretching as needed
Your PT will progress your exercises as you get stronger. What starts as gentle stretches will build toward more challenging, functional movements.
Tips for Getting the Most Out of PT
- Do your home exercises. This is the #1 factor in how fast you recover.
- Ask questions. If you don’t understand why you’re doing something, ask.
- Track your progress. Notice what’s better and what’s still hard. Share this with your PT.
- Be patient. Some conditions take weeks to improve. Trust the process.
- Communicate pain honestly. Don’t push through sharp pain, but don’t avoid all discomfort either.

“Will it hurt?” The evaluation shouldn’t be painful. Treatment might cause some temporary discomfort, especially with manual therapy or stretching. Your PT will check in with you throughout.
